
Your Personal Data
The personal data which you provide when completing the Staywell questionnaire will be used by the NHS health team providing your care. This will usually be your GP or the nurses at your practice. It may extend to the medical staff in your hospital if you ever have to be admitted, or to other approved, community care workers who look after you.
Their purpose in seeing the information you have provided is to learn where you may need some extra healthcare support. For example, you may have said you are not sure about some of the best ways of managing your medical condition or of taking your medication. Staywell is designed to help them to help you more.
Your personal data is made available to your care team via a secure internet link. Only authorised users can look at your data and they have to enter their own unique user name and password to do so.
Your data is stored in London and protected by an organisation* which is a world leader in this field. For example, they have been authorised by the Ministry of Defence to store details of military personnel on active duty.
A few authorised technicians within this London organisation and within the organisation** who runs the Staywell system on a day to day basis, may come into contact with your data while doing maintenance work. They are all carefully vetted and contracted not to extract or divulge data to any unauthorised person. In addition, your data is held in a “scrambled” form so that it is very difficult for anyone without the “key” to make sense of it.
Your data is protected from outside interference and theft by the latest computer software designed for this task.
